How do I create a survey?

Create a survey using a variety of questions types, pages, and logic to collection detailed information for assessments or recipient surveys.

Introduction

Surveys have three phases:

1. Survey Template - Templates are used a boiler plate for surveys and do not track responses.
2. Activated Survey - An instance of a survey template that tracks responses.
3. Deactivated Survey (Coming Soon) - A survey that has been deactivated will not longer accept responses. It is read-only.
   
   

Prerequisites

• Required Permissions: Manage Surveys & View Handweave 

Create a Survey Template

1. Navigate to https://apps.alertsense.com/#/surveys
2. Click Manage Templates
3. Click the "+" icon in the upper right corner. 
   or
   Navigate to an existing survey and user the Create a Copy to base your new survey on an existing template.
4. Give the survey template a Name and Description.
5. Use the Designer tab to build your survey questions. Please refer to the SurveyJS documentation for support on survey design.
6. Use the Preview tab to test your survey. While is Preview your responses will not be recorded.
7. Click the Save button to save your survey template. 

Activate a Survey

1. Navigate to https://apps.alertsense.com/#/surveys
2. Click Manage Templates
3. Click on the survey you wish to activate
4. Click the Activate button in the upper right corner.
5. The survey is now available for responses.
